I am relocating to Texas for warmer weather! I will be closing my practice in New York in May, 2010.

My Mount Kisco office will no longer be open as of May 8, 2010. The main phone number, (914) 666-6609, will continue to be available for you to request a copy of your medical records if required.

To obtain a copy of your medical records, please follow the steps below:

  1. Click on the following link to download the Authorization to Release Medical Records. This may be used either to receive a copy for yourself or have it sent to another physician (it may be easier for you to receive it and provide it to another physician yourself—that way you’ll always have your own copy).
  2. Fill out form and indicate one method of receipt for records (fax or regular mail).
  3. Fax form to (914) 206-4450.

If you should require care in the future, click here to obtain a list of local plastic surgeons, or access Northern Westchester Hospital's website. There you can click on “Find a Physician”, to access the Physician Referral page, and select Plastic Surgery under “Specialty”. A list of qualified, local Board-Certified Plastic Surgeons can be found there. Alternatively, you may call NWH at (914) 666-1200 and request a referral.

You can also contact the Westchester County Medical Society at (914) 967-9100 for a list of local, Board-Certified Plastic Surgeons.
